Some guidelines for filling up the DS-160 form:
• Applicant’s Social Security Number (SSN) and driver’s license information (applicable those who have been to the US.• Security related questions.
• Detailed information regarding previous visa applications.
• Full name and immigration status of all immediate relatives in the US.

Country whose citizen you are. e.g. “India”

If applicable, or select “Does not apply”
THE MOST IMPORTANT
Home Address
Complete home address should be the present address where you are staying presently. It can differ from your permanent address mentioned in your passport. If you are currently in USA and would like to come on vacation to home country (such as India) for stamping, please provide either your relative's or friend's address.
Mailing Address
Mailing address for the passport should be always:
Your address to which the passport will be sent after visa stamping.
Work Phone Number
Mention Your office phone number.
Work Fax Number
Mention your office fax number. (if none then mention n/a)
Passport Number
Please mention only current passport number. Please do not mention which has already expired, cancelled or lost.
Purpose of your trip
Choose from the drop down menu in case of B1 Visa.
Intended Date of Arrival
Either be a specific date or a tentative date
Intended Length of stay in US
Select 4 weeks in case of B1.
Address where you will stay in the US
Provide the address where you will stay or you can provide your RM/BRM address and Phone Nos/Mobile nos. Address should match with the client location mentioned in the petition.
Person/Entity paying for your trip
Select “Company/Organization” from the drop down menu and write “your company name and phone number with the required extension”
Security and Background Medical and Health Information
If “Yes” please explain or click “No”
Where do you intend to work?
Please provide the complete address
In case of B1/L2/H4 visas, say “No”.
